Indication.
– An additional tool in the complex treatment of acute and chronic rheumatoid arthritis, osteoarthritis, osteoarthritis, ankylosing spondylitis, psoriatic polyarthritis, gouty arthritis;
– localized forms of extra-articular rheumatism of the musculoskeletal system: tendonitis, synovitis, tendovaginitis, inflammation of fascia and joint ligaments, bursitis;
– local symptomatic treatment of inflammation, pain and swelling in post-traumatic lesions of the musculoskeletal system – sprains, dislocations, bruises.
Contraindication.
Increased sensitivity to the components of the drug. Hypersensitivity to aspirin or other nonsteroidal anti-inflammatory drugs (NSAIDs) with clinical manifestations of an asthma attack, urticaria, or allergic rhinitis. Gastric and duodenal ulcer disease, ulcerative colitis, bronchial asthma, rhinitis, conjunctivitis or bronchospasm while taking NSAIDs, hemophilia, hypocoagulation, glucose-6-phosphate dehydrogenase deficiency, blood diseases.
Method of application and dosage.
For adults and children over 14 years of age, apply 2.5 g of ointment to the affected area (a column of ointment 7.5 cm long) 2 times a day in a thin layer, lightly rubbing into the skin. To increase the effectiveness of therapy, it is possible to use occlusive bandages, as well as physiotherapeutic methods (high-frequency ultrasound therapy).
The average daily dose for adults and children over 14 years of age is 5 g of ointment (250 mg of indomethacin). Duration of treatment — 7-10 days. If there is no effect on symptoms during this time period, treatment should be reconsidered. With long-term treatment (more than 10 days), laboratory control of the number of leukocytes and platelets is recommended.
Children.
Do not use in children under 14 years of age.
Overdose.
Symptoms: dyspepsia, erosive and hemorrhagic lesions of the gastrointestinal tract, headache, dizziness, hepatotoxicity. It is necessary to monitor the leukocyte and platelet formula.
Treatment: cancel the use of the drug or reduce its dose. There is no specific antidote, the therapy is symptomatic.
Symptoms in case of contact with eyes, mucous membranes or open wounds: local irritation is observed – lacrimation, redness, burning, pain.
Treatment: copious washing of the affected area with distilled water or physiological solution until the complaints subside.
In case of accidental ingestion, a burning sensation in the oral cavity, salivation, nausea, and vomiting may occur. In such cases, it is necessary to rinse the oral cavity and stomach, if necessary – to carry out symptomatic treatment.
To prevent an overdose of the drug, it is necessary to strictly follow the prescribed treatment regimen.
Adverse reactions.
Adverse reactions are related to the method of application.
The probability of systemic side effects with local use of indomethacin is insignificant compared to the frequency of side effects with oral use.
From the side of the skin and subcutaneous tissue: sometimes with local application in susceptible patients, small-point and erythematous-papular rashes, itching, hyperemia and burning at the application site, peeling, dry skin, edema can be observed.
From the side of the respiratory system: shortness of breath.
From the side of the hepatobiliary system: an increase in the levels of liver enzymes.
From the side of the immune system: local edema and vesicular rash rarely develop, and in some cases – angioedema, a hypersensitivity reaction.
From the gastrointestinal tract: when applied to a large surface of the skin, resorptive side effects may occur, for example, disturbances from the gastrointestinal tract (including loss of appetite, nausea, vomiting, diarrhea, pain, hemorrhages and ulcers), increase in the level of liver enzymes.
With long-term use of the drug or application to a large surface of the skin, systemic side effects may occur: dyspepsia, headache, dizziness.
In such cases, the use of the drug should be stopped.
