Clinical characteristics.
Indication.
Symptomatic treatment of fever and pain of various origins in children aged 3 months to 12 years with a body weight of at least 5 kg (including fever after immunization, acute respiratory viral infections, influenza, teething pain, pain after tooth extraction, toothache, headache pain, sore throat, pain when sprained ligaments and other types of pain, including inflammatory genesis).
Contraindication.
Hypersensitivity to ibuprofen or any of the excipients of the drug.
History of hypersensitivity reactions (eg, bronchospasm, bronchial asthma, rhinitis, angioedema, or urticaria) after taking acetylsalicylic acid (aspirin) or other NSAIDs.
Active peptic ulcer disease/bleeding or history of recurrences (two or more severe episodes of confirmed peptic ulcer disease or bleeding).
History of gastrointestinal bleeding or perforation associated with NSAID use.
Severe heart failure, severe liver failure or severe kidney failure.
· Severe dehydration (caused by vomiting, diarrhea or insufficient fluid intake).
· The last trimester of pregnancy.
Cerebrovascular or other bleeding.
Disorders of hematopoiesis of unknown etiology or blood coagulation.
Hereditary fructose intolerance.
Overdose.
In children, overdose symptoms may occur when taking a dose of ibuprofen exceeding 400 mg/kg. In adults, the dose response is less pronounced. The half-life in case of overdose is 1.5–3 hours.
Symptoms In most patients, the use of a clinically significant amount of NSAIDs caused only nausea, vomiting, pain in the epigastric area, less often – diarrhea. Tinnitus, headache and gastrointestinal bleeding may also occur. With more severe poisoning, toxic damage to the central nervous system is possible in the form of vertigo, dizziness, drowsiness, sometimes – an excited state and disorientation or coma. Sometimes patients develop convulsions. In severe poisoning, hyperkalemia and metabolic acidosis may occur, as well as an increase in prothrombin time/MIV (probably due to interaction with circulating coagulation factors). Acute renal failure, liver damage, hypotension, respiratory depression, and cyanosis may occur. In patients with bronchial asthma, an exacerbation of the course of asthma is possible. Nystagmus, blurred vision and loss of consciousness are possible.
Treatment. There is no specific antidote. Treatment should be symptomatic and supportive, as well as include ensuring the patency of the respiratory tract and monitoring cardiac function and vital signs until the patient’s condition normalizes. The need for oral use of activated charcoal or gastric lavage should be considered if no more than 1 hour has passed after the patient has taken a potentially toxic dose of the drug. If ibuprofen has already been absorbed, you can use alkaline substances that contribute to the excretion of acidic ibuprofen with urine. For frequent or prolonged seizures, intravenous diazepam or lorazepam should be prescribed. In case of exacerbation of bronchial asthma, bronchodilators should be prescribed.
